Life at Home —
Transition and Children’s Services (T&C)
Provided by our Residential Team
Every child deserves a safe, stable, and nurturing environment where they can grow, learn, and thrive. Our Transition & Children’s Services are designed to meet the specific needs of children and young adults, with a focus on building skills, fostering confidence, and creating opportunities for a bright future. Guided by person-centered values and a deep commitment to self-determination, we work collaboratively with children, their families, and their support teams to ensure every child has the tools they need to lead a fulfilling life.
Our Philosophy of Service
We recognize that self-determination is progressive and evolves with age and experience. For children, this means helping them develop their ability to make choices, express themselves, and gain independence, always in partnership with their family and support team. Our approach is tailored to the unique needs and goals of each child, prioritizing compassion, patience, and a commitment to their well-being.
We believe that children are best served at home with their families. When this is not possible, our goal is to provide a home that offers safety, comfort, and stability while fostering a sense of belonging and community. We strive to create a setting that feels as much like a family home as possible, ensuring each child feels valued and supported.
To achieve this, we are guided by our Transition and Children’s Services Advisory Group, a dedicated team of professionals with diverse expertise who are responsible for designing and enhancing these services. Their contributions include developing specialized training, establishing and improving processes that ensure consistent, high-quality services, and staying informed on the best practices for support.
Our ultimate goal is to help children build a strong foundation for adulthood, equipping them with the skills, relationships, and confidence to pursue the life they choose.
Our Beliefs and Commitments
Children thrive with a foundation of compassion, patience, and individualized attention.
Every child is unique, and their support must be customized to their specific needs and goals.
Family relationships are essential, and children must be safely connected and free to strengthen those bonds.
Growth comes from exposure to new opportunities and challenges, even with some inherent risk.
Our approach is person-centered, culturally aware, and tailored to each child’s age, development, and unique needs.
Our approach always starts with the least restrictive option.
We have a focus that aligns with the child’s natural growth stages and prepares them for the future.
Play, community connection, and educational opportunities are vital for growth and development.
A child’s home should foster trust, belonging, and understanding with clear boundaries and personal agency.
We have a commitment to building confidence, genuine relationships, and a sense of belonging in their community.
As an organization dedicated to learning and evolving, we continuously incorporate new and emerging practices to ensure the highest quality of care, including a clinically informed and evidence-based approach. We work hand-in-hand with families, schools, and other key partners to create a collaborative plan that supports each child’s journey.
